What is the Free PSA test?

The free PSA test is a blood test that measures the amount of prostate-specific antigen (PSA) present in the free/unbound state in your blood. This protein is produced by the prostate glands below the kidneys and can be free or bound to other proteins. PSA plays a role in reproductive health by breaking down larger proteins in semen, reducing its thickness and aiding in sperm movement to improve fertility. Under normal conditions, there is a trace amount of this protein present in the blood. However, certain conditions like prostate cancer, an enlarged prostate, inflammation (prostatitis), or even physical trauma can increase PSA levels. Elevated PSA levels can result from benign conditions like an enlarged prostate or ageing. To distinguish between cancer and non-cancerous issues, doctors may measure free PSA levels in men with PSA between 4 ng/mL and 10 ng/mL to decide if a biopsy is needed. Analysing free PSA percentage helps assess prostate cancer risk.
