  • 4This Card can be used only for full payment by Cash/Card/UPI.
  • 5(S)” Marked (Special Tests) in Bill would not be eligible for discount.
  • 6Home visit timings will be scheduled according to availability of staff.
  • 7Home visit Charge will be Applicable As Per Rules.
  • 8Some promotional offers may not be combined with this card.

  • 10This card will not be replaced when lost, damaged or stolen.
